This form is for new OSSE-licensed facilities requesting to get set up in SmartTeach so a designated Quorum administrator can view, add, remove, and edit staff members once access is approved. By submitting this form, you are requesting the creation of a new center in SmartTeach and an OSSE-sponsored administrator account to manage staff enrolled in Quorum eLearning professional development courses.
